Overview
Social Worker – Medical Social Worker, Everett, WA. Start date 06/08/2026 for a 13-week travel assignment in a hospital setting. Weekly pay range: $1,708 – $1,829 per week. Guaranteed hours: 72 hours per 2-week pay period. Shift: 5x8 Days. The role supports adult and geriatric patients through discharge planning and care coordination; prior acute care and care transitions experience is valued. Education pathway includes MSW options and accredited WA licensure.

Responsibilities
- Perform biopsychosocial assessments and needs assessments to guide care planning for adults and geriatrics.
- Lead discharge planning and care coordination, including referrals to home health, DME, andPlacement to rehab or long-term care as appropriate.
- Document the plan of care and progress in the electronic health record, ensuring timely updates and care transitions.
- Collaborate with an interdisciplinary team (including PT, pharmacy, interpretation services, social services, transportation) to support patients’ transitions from hospital to next level of care.
- Support patients with complex medical and social needs, contributing to appropriate placement and follow-up across Med-Surg, ED/ICU settings as needed.
- Maintain compliance with hospital policies and licensing requirements and participate in unit-based education and orientation activities.

Requirements
- Licensure: Washington social work licensure required (LICSW, LSWAIC, LASW, LSWAA); facility also accepts LMSW or LCSW with Master’s degree; WA license in-hand preferred for unit eligibility (per compliance notes).
- Certifications: BLS required.
- Experience: Minimum 1 year in acute care, discharge planning/care coordination; experience with case management across Med-Surg, long LOS, and ED is preferred.
- Education: Master’s degree in Social Work (MSW) or equivalent; RN with relevant discharge planning experience may be considered.
- Skills: Epic charting experience required; ability to work in a fast-paced hospital environment; strong interpersonal and interdisciplinary collaboration skills.
- Schedule & environment: Able to work the stated shift pattern (Days, 5x8) and to commit to every-other weekend; willing to float across hospital areas as needed; orientation and pre-start training required.
